Barry Keoghan has drawn attention with a new social media video that offers a behind-the-scenes look at a cosmetic skin treatment. The clip was shared by Nilam Holmes, a U.K. celebrity facial expert and the founder of Dermaspa, and shows the Oscar nominee receiving a Xerf session.

According to the Dermaspa website, Xerf does not involve injections. It relies on radio-frequency technology to tighten the skin and renew collagen. A session for the face and neck costs around $3,500.

In her caption, Holmes said she has worked with Keoghan for some time, caring for his skin across different films, campaigns and projects. "His skin has really improved over the last few years and he looks great," she wrote. She said the video showed his second Xerf skin-tightening treatment. Holmes explained that she was running late while filming other Xerf content, so Keoghan offered to let her film him undergoing the procedure instead of rushing her.

Keoghan narrates the video himself. "I've been hearing a lot about it, and Nilam is very excited about it, and she's great at what she does," he says. He describes each visit as relaxing and therapeutic, adding: "I feel better about myself; I look better. I think it's important to look after yourself inside and outside."

Holmes' social media pages show that other clients who have had Xerf treatment include Chris Appleton, Rosie Huntington-Whiteley, Joe Alwyn and Nicole Scherzinger. She has also worked with Pink. She has regularly posted about treating Keoghan, including facials ahead of various photo shoots.

In March 2025, Holmes wrote about preparing Keoghan for his Wonderland magazine cover. She said that working on movies, campaigns, interviews, press and philanthropy projects brings an unstructured routine, travel, makeup and disrupted sleep, all of which take a toll on the skin. Their regular sessions, she said, involve fixing problematic skin, calming inflammation, maintaining good skin and aiming for a healthy, rested look. She also described Keoghan as passionate and eager to learn, saying he "brings the fun" when he visits.

The video arrives months after Keoghan went viral for comments on SiriusXM's "The Morning Mash Up." He said online "abuse" about his appearance had become so severe that he no longer wanted to "go outside."

"There's a lot of hate online," Keoghan said at the time. "There's a lot of abuse of how I look, and it's kind of past the point of, you know, everyone goes through that, but it's made me shy away. It's made me really go inside myself, not want to attend places, not want to go outside." He added that he was being "absolutely pure and honest" and that "it's becoming a problem."

Keoghan said he had removed himself from social media to avoid unpleasant comments, though he occasionally goes online after a premiere or event to see how he is received. Most of the time, he said, the reaction is "not nice."
